Supporting your health care journey
Nurse Navigators are a team of registered nurses and Indigenous Nurse Navigator Support Officers. We focus on your entire health care journey and all your health needs, not just on a specific disease or condition.
As experienced nurses, we have clinical knowledge and understand the health system. This means we know what your health care needs are and we can link you to the services that are right for you.
We can also help by:
- developing care plans with you
- being your key point of contact
- liaising with your other care providers
- scheduling and going with you to your appointments.
We'll help you understand your medical conditions and know what your options are.
Who can get help
Nurse Navigators care for patients with complex health conditions who need a high level of clinical care.
This may include patients with multiple chronic illnesses, those with a high need for health services, or those who have complicated health conditions.
Our teams are located across the region for patients living in Cape York, Northern Peninsula Area and Torres Strait Islands.
Finding a Nurse Navigator
If you have complex health conditions, your GP or other health provider will contact us on your behalf if you'd like them to.
